Photo & Video Release

I, the individual identified below, hereby grant Lucky Cats Rescue, Inc. (the “Company”)  and its assigns, successors in interest, employees, consultants, and those acting on behalf of the Company, the irrevocable unrestricted right to (i) use my name (or any fictional name) and (ii) use my picture, portrait, voice, image and likeness in connection with, and as depicted in, all pictures, photographs (including without limitation all originals, negatives, prints and transparencies), audio recordings and/or video recordings in any and all forms, media and manners now or hereafter known (collectively, “Images”), without restriction as to changes or alterations, for advertising, promotion, exhibition, or other lawful purposes.  I acknowledge and agree that I have no right, title, or interest in any of the Images.  I waive any right to inspect or approve any Images produced or any finished products incorporating my name and/or the Images.  

I hereby release and hold harmless the Company from any damages or liability relating to or arising from any use of or modification or alteration to any of the Images.  I waive any claim I may have based on any use of the Images or works derived therefrom, including but not limited to claims for invasion of privacy or libel.  I understand that the Company would not use the Images without first receiving a copy of this Release signed by me or by my parent or legal guardian, as set forth below.  

I represent and warrant that I have read this Release and am familiar with its contents. I further represent and warrant that I am either (i) 18 years or older and competent to sign this release, or (ii) in the case of an individual under the age of 18, a parent or legal guardian of the below-identified individual with the authority to enter into this Release on behalf of such individual.  This Release shall survive the execution and delivery hereof and shall be binding on me, my legal representatives, heirs and assigns.
